Referencing healthd can cause problems on EB instances that do not
have enhanced logging enabled. This commit removes it, allowing the
configuration to work regardless of whether enhanced logging is
enabled.

This commit adds a way to differentiate a local failure from a "early
exit" triggered by a failure on another node. Not separating this can
cause a race condition when two nodes fail at roughly the same time.

Bind to all available interfaces -- seems to fix an issue where a metabase container can't be accessed from multiple docker networks.

This unifies the code and the environment variables for specifying
test database connection parameters across all supported
databases. Existing databases that supported environment variables for
tests have been refactored to use the new environment variable
names. All of the connection environment variables are named similar
to:

MB_<DBNAME>_TEST_<PARAM>

For user on MySQL, the variable would be named:

MB_MYSQL_TEST_USER

Allow the AWS elb to run the health check over http
when https is enabled.
Make disabling forces TLS possible.
Simplify nginx config handling on beanstalk by providing
the config directly rather than editing persistent files in place
using sed.

If the DB is using H2 and it's stores outside of docker, then we need
to be polite and safe about how we set permissions on the directory.
